Job summary

We are seeking an enthusiastic and experienced Paramedic Practitioner to join our Acute Home Visiting Service, delivering high-quality care to patients in their own homes across the Primary Care Network (PCN).

Working autonomously within a multidisciplinary team, you will assess, diagnose, treat and manage patients with acute illness, frailty and complex health needs who are unable to attend surgery. Using advanced clinical skills, you will develop management plans and facilitate admission, referral or community-based care where appropriate.

This is a part-time role working 10:00am to 6:00pm. Working days will include every Friday, which is fixed due to service demand, together with either Wednesday or Thursday, to be agreed with the successful candidate.

Working closely with GPs, community teams, care homes and secondary care services, you will deliver integrated care, support patients to remain safely at home and help prevent avoidable hospital admissions. You will also contribute to service development, quality improvement and patient education.

The successful candidate will be expected to complete the First Contact Practitioner (FCP) Roadmap to Practice if not already achieved. Full support, supervision and development opportunities will be provided to enable completion of the pathway.

Interviews are expected to be held on the 12th October and 14th October 2026.

Main duties of the job

The Paramedic Practitioner will provide autonomous, patient-centred care within the Primary Care Network, delivering acute home visits and supporting patients within both community and practice settings. Working as part of the multidisciplinary team, the postholder will assess, diagnose, treat and manage patients presenting with acute and long-term health needs, developing appropriate management plans and facilitating referral where required. The role includes undertaking urgent home visits, avoiding unplanned admission reviews, prescribing within competence, supporting vaccination programmes and chronic disease management for housebound patients. The postholder will promote self-care and patient education, maintain accurate clinical records, participate in audit and quality improvement activities, and work collaboratively with colleagues to ensure safe, effective, evidence-based care.

The Paramedic Practitioner will work as an autonomous clinician within Central and West Warrington Primary Care Network, delivering an Acute Home Visiting Service to patients who are unable to attend their GP practice due to acute illness, frailty, mobility limitations, or complex healthcare needs.

Working as part of a multidisciplinary team, the postholder will provide high-quality, patient-centred care through comprehensive assessment, diagnosis, treatment, and management of patients within their own homes. The role will support patients to remain safely within the community where appropriate, reduce avoidable hospital admissions, and contribute to proactive population health management across the PCN.

The postholder will also support vaccination programmes and undertake proactive chronic disease reviews for housebound patients, ensuring patients receive timely, holistic, and evidence-based care. Training will be provided where necessary and support will always be available when needed.

Main Duties and ResponsibilitiesClinical Responsibilities

• Undertake urgent and same-day home visits for patients requiring assessment and management within the community.
• Assess, investigate, diagnose, treat, and manage patients presenting with acute and complex health conditions.
• Develop and implement appropriate management plans and facilitate referral, escalation, admission avoidance, or community-based treatment where appropriate.
• Triage and manage acutely unwell patients within the community setting.
• Carry out Avoiding Unplanned Admission (AUA) reviews and support proactive care planning for vulnerable patients.
• Prescribe or issue medications within scope of practice and in accordance with local policies, clinical pathways, and prescribing regulations.
• Discuss patients requiring referral to secondary care with a supervising GP prior to referral where appropriate.
• Maintain accurate, contemporaneous, and comprehensive clinical records.
• Ensure clinical practice remains safe, effective, evidence-based, and within professional boundaries and scope of competence.

Vaccination Programmes

• Support the planning and delivery of vaccination programmes, including seasonal influenza, COVID-19, and RSV vaccination campaigns.
• Undertake vaccination visits for eligible housebound patients.
• Promote vaccine awareness and uptake through patient education and health promotion activities.
• Work collaboratively with practices and PCN colleagues to improve vaccination coverage amongst vulnerable patient groups.
• Ensure vaccines are administered safely and in accordance with national guidance and local protocols.

Chronic Disease Management

• Undertake proactive chronic disease reviews for housebound patients within their own homes.
• Support the assessment, monitoring, and management of long-term conditions including diabetes, COPD, asthma, hypertension, heart failure, and frailty.
• Work alongside GPs, Practice Nurses, Clinical Pharmacists, and community teams to optimise patient care and improve health outcomes.
• Contribute to personalised care planning, medication optimisation, and preventative healthcare interventions.
• Identify clinical deterioration or unmet healthcare needs and initiate appropriate management, referral, or escalation.
• Support delivery of Quality and Outcomes Framework (QOF) objectives and long-term condition management programmes.

Patient Education and Health Promotion

• Provide health education, self-management advice, and support to patients, carers, and families.
• Empower patients to make informed decisions about their treatment and ongoing healthcare needs.
• Promote health prevention initiatives and wellbeing interventions where appropriate.

Multidisciplinary Team Working

• Act as a specialist Paramedic resource for healthcare professionals and service users across the PCN.
• Work collaboratively with GPs, Practice Nurses, District Nurses, Clinical Pharmacists, Care Coordinators, Social Prescribers, Community Services, Care Homes, and Secondary Care Providers.
• Participate in multidisciplinary team meetings and case discussions.
• Communicate effectively with colleagues and external organisations to ensure continuity and coordination of care.

Quality, Governance and Service Development

• Participate in clinical audit, service evaluation, quality improvement initiatives, and implementation of best practice.
• Contribute positively to service development and innovation across the PCN.
• Work within all organisational policies, procedures, and clinical governance frameworks.
• Maintain HCPC registration and fulfil Continuous Professional Development (CPD) requirements.
• Participate in peer review, supervision, appraisal, and mandatory training.
• Support and share knowledge with colleagues to promote learning and continuous improvement.

Staff partaking in training that meets certain requirements will be required to sign a Training Agreement that outlines associated costs to the training. Staff may be liable for reimbursing those costs to the Practice / PCN if they leave their PCN role before, during or after the training.
