Health & Wellbeing Coach – exercise referral specfic


We are looking for an enthusiastic and experienced Health & Wellbeing Coach to support individuals referred through primary and secondary care as well as community and voluntary sector partners. This role focuses on supporting people living with long-term health conditions to safely increase their physical activity levels, improve their wellbeing, and achieve positive health outcomes, whilst helping people to develop the knowledge and confidence to manage their health independently.
The post will involve working with specialist populations who are living with, or at risk of developing, long-term conditions. Experience of supporting individuals with cardiovascular conditions, delivering community cardiac rehabilitation and falls prevention programmes, is highly desirable.


The successful candidate will have experience of delivering physical activity interventions for specialist populations within community settings, including gyms, leisure centres and swimming pools. They will be confident in conducting assessments, developing tailored activity plans, and supporting behaviour change to help individuals achieve sustainable improvements in their health and wellbeing.


The role includes maintaining accurate records, monitoring health outcomes, and contributing to service reporting requirements.
The successful candidate will be committed to delivering inclusive services that reflect the diversity of Blackburn with Darwen, and helping to reduce health inequalities through accessible, community-based health and wellbeing programmes. The role may also involve supporting the delivery of community health checks and wider health improvement initiatives across the service.


Key requirements:
• Level 2 Gym Instructor qualification (essential)
• Level 3 Exercise Referral qualification (essential)
• Experience working with GP, physiotherapy and secondary care referrals
• Knowledge and experience of supporting people with long-term health conditions
• Experience in delivering physical activity programmes on pathways such as cardiac rehabilitation, falls prevention or stroke rehabilitation are strongly desirable.
• strong administration, record-keeping and outcome reporting skills.
• Ability to work flexibly across a range of community venues and leisure centres including gyms and pools.
• Experience of using monitoring, reporting, or case management systems to record participant engagement and outcomes.
• Good IT skills, including proficiency in Microsoft Office applications and data management systems.
